<p>Tách ra từ Trung tâm y tế thành phố Tuy Hoà vào năm 2002. Bệnh viện Đa khoa huyện Phú Hoà được thành lập theo quyết định số 1622/2005/QĐ UBND tỉnh Phú Yên,</p>
